Continuous Improvement personnel develop COAs to take action on observations. Effective COAs should:

• Be based on validated observations.
• Be reviewed by subject-matter experts.
• Be developed in coordination with and assigned to a responsible entity for implementation.
• Include a clearly defined timeline for completion.
